Member Documents

The “Documents” tab of longitudinal record enables users to view all documents uploaded and associated with a specific member as well as the document type and date uploaded.

Clicking the link for the document name enabled the user to download and view the document.

New documents can be uploaded by clicking the “Upload Document” button in the upper right corner of the panel.

When uploading documents, the user is required to:

  1. Specify a name for the document
  2. Choose a Type category for the document
  3. Choose the document to be uploaded

Optionally, the “Available in Portal” checkbox can be selected to provide visibility to the document within the care team portal.

Clicking the “Save” button will upload the document.
